ABSTRACT

Bronchial asthma(asthma)is a common chronic airway inflammatory disease in children, most of which are allergic asthma.Allergen immunotherapy can change the natural course of asthma, and has certain efficacy in controlling asthma symptoms, reducing airway hyperresponsiveness and reducing the use of control drugs.It is the treatment for the cause.The most common allergen immunotherapy treatments are subcutaneous immunotherapy and sublingual immunotherapy.This article reviews the effectiveness of subcutaneous immunotherapy in the treatment of children with asthma, and focuses on the effective evaluation indicators and potential biomarkers that can be used as reference in clinical practice.

ABSTRACT

Objective:To explore the impact of PM 2.5 concentration in Shanghai on the incidence of allergic rhinitis(AR) in the population, and provide strategies for early warning and prevention of AR. Methods:Collect daily average concentrations of atmospheric pollutants monitored in Shanghai from January 1, 2017 to December 31, 2019, and clinical data of AR patients from five hospitals in Shanghai during the same period. We used a time-series analysis additive Poisson regression model to analyze the correlation between PM 2.5 levels and outpatient attendance for AR patients. Results:During the study period, a total of 56 500 AR patients were included, and the daily average concentration of PM 2.5 was(35.28±23.07)μg/m³. There is a correlation between the concentration of PM 2.5 and the number of outpatient attendance for AR cases. There is a positive correlation between the daily average number of outpatient for AR and levels of PM 2.5 air pollution((P<0.05)) . We found that every 10 μg/m³ increase in PM 2.5, the impact of on the number of AR visits was statistically significant on the same day, the first day behind, and the second day behind, with the strongest impact being the exposure on the same day. Every 10 μg/m³ increases in PM 2.5, the number of outpatient visits increased by 0.526% on the same day(95%CI 1.000 50-1.010 04). Conclusion:The atmospheric PM 2.5 concentration in Shanghai is positively correlated with the number of outpatient for AR, and PM 2.5 exposure is an independent factor in the onset of AR. This provides an important theoretical basis for AR.

ABSTRACT

Objective:To analyze the status and influencing factors of long-term care demand of the community elderly in Taian and in order to provide a theoretical basis for the government to formulate policies to meet the needs of the elderly.Methods:A self-designed questionnaire was used to investigate 294 elderly aged 60 years in three areas in Tai'an using Convenient sampling.Results:The total average score of long-term care demand was (2.67±0.46). The top three demand are economic, chronic disease rehabilitation, and dignity demand. Multiple linear regression showed that age, living conditions, place of residence, and chronic disease were the influencing factors of long-term care needs ( P<0.05). Conclusions:The long-term care needs of the elderly in Tai'an community were at a medium level, Age, living conditions, place of residence and chronic diseases have different effects on long-term care demand. It is suggested that the government and medical staff should pay more attention to the economic, rehabilitation and dignity needs of the elderly in the community, and formulate and implement relevant policies to meet the needs of the elderly in order to improve the quality of life and happiness of the elderly.

ABSTRACT

Inulin has been used as a prebiotic to alleviate glucose and lipid metabolism disorders in mice and humans by modulating the gut microbiota. However, the mechanism underlying the alleviation of metabolic disorders by inulin through interactions between the gut microbiota and host cells is unclear. We use ob/ob mice as a model to study the effect of inulin on the cecal microbiota by 16S rRNA gene amplicon sequencing and its interaction with host cells by transcriptomics. The inulin-supplemented diet improved glucose and lipid metabolism disorder parameters in ob/ob mice, alleviating fat accumulation and glucose intolerance. The α diversity of gut microbial community of ob/ob mice was reduced after inulin treatment, while the β diversity tended to return to the level of wild type mice. Interestingly, Prevotellaceae UCG 001 (family Prevotellaceae) was obviously enriched after inulin treatment. A comparative analysis of the gene expression profile showed that the cecal transcriptome was changed in leptin gene deficiency mice, whereas the inulin-supplemented diet partially reversed the changes in leptin gene-related signaling pathways, especially AMPK signaling pathway, where the levels of gene expression became comparable to those in wild type mice. Further analysis indicated that Prevotellaceae UCG 001 was positively correlated with the AMPK signaling pathway, which was negatively correlated with markers of glycolipid metabolism disorders. Our results suggest that the inulin-supplemented diet alleviates glucose and lipid metabolism disorders by partially restoring leptin related pathways mediated by gut microbiota.

ABSTRACT

Objective This paper reviews the research context of the fear of cancer recurrence abroad, and presents the research hotspots and frontiers in this field, which will further provide references for the research of the fear of cancer recurrence in China. Methods Web of Science database was retrieved,the time of papers published, the number of publications and periodicals were analyzed.The Map drawing was accomplished with the help of Histcite software. Results Total 5322 effective literatures were investigated,most of the papers are published by the United States, Butow P was the high yield authors. and the core journal is "PSYCHO-ONCOLOGY". High frequency key words mainly concentrated on cancer, breast cancer, prostate cancer, quality of life, prognosis, chemotherapy (symptom distress and management). Conclusion In the aspect of fear of cancer recurrence, Europe and America are in a leading position, compared with the international level,there is still a gap in the field of FCR in China , the depth and breadth of research needs to be extended further, We should keep path with the international FCR research hotspot, to broaden our research scope and improve the quality of related research.

After the development of domestic embedded subject service and its domestic and foreign main models were described, following suggestions were put forward: thinking approach teaching contents and multiple elementary assessment system should be designed based on OBE theory with medical postgraduate general practical medical literature novelty assessment course as its starting point, excellent students should be trained as scientific information detectors by improving their information literacy and practical ability through small scale blended teaching, the professional bottleneck of librarians and talent team construction should be solved by cultivating subject service team for different subjects and strengthening the contacts between different subjects.

Methods 80 patients with idiopathic facial palsy were evaluated with the House-Brackmann (H-B) scale and SFGS before and every 2 weeks after treatment until 12 weeks or recovery. They were assessed by 2 testers before and 2 weeks after treatment. The results were analyzed and compared. Results The Cronbach's α coefficient was 0.93 of SFGS, and 0.74 of H-B scale. The intraclass correlation coefficient (ICC) between testers was 0.84 or higher of SFGS, and 0.77 or higher of H-B scale. The Spearman's rank correlation coefficient between SFGS and H-B scale was 0.73, Kappa coefficients was 0.81 for grade II, 0.74 for grade III, 0.49 for grade IV, 0.66 for grade V, and 0.92 for grade VI. The scores of SFGS and H-B scale positively correlated with the recovery time. Conclusion SFGS is a kind of evaluation for peripheral facial paralysis with satisified reliability and validity, which can be applied to evaluate the severity and outcome during acute stage.

OBJECTIVE@#To investigate the correlation of polysomnography parameters and CT measurements in upper airway of mild and severe obstructive sleep apnea hypopnea syndrome (OSAHS) patients before and after uvulopalatopharyngoplasty (UPPP).@*METHOD@#Having PSG detection and spiral computed tomograph scan for 30 mild and severe OSAHS patients both before and after UPPP operation, compare the morphology change of upper airway on CT measurements, use pearson correlation analysis to analysis the correlation between the minimum cross-sectional area, left and right diameter, anteroposterior diameter in upper airway and apnea hypopnea index (AHI).@*RESULT@#The difference of the minimum cross-sectional area, left and right diameter, anteroposterior diameter in upper airway before and after UPPP operation was significant. The minimum cross-sectional area, left and right diameter was negatively correlated with AHI; Left and right diameter was not correlated with AHI.@*CONCLUSION@#The minimum cross-sectional area, left and right diameter, anteroposterior diameter after operation is bigger than before operation. The minimum cross-sectional area, left and right diameter is negatively correlated with AHI.

ABSTRACT

OBJECTIVE@#To investigate relationship between anti-endothelial cell antibody(AECA) and response to dexamethasone in sudden hearing loss(SHL).@*METHOD@#Forty-eight SHL patients and thirty normal controls with SHL were recruited in present study. AECA was detected by ELISA in serum of all normal controls and SHL patients as well as pure-tone average was examined by electronic audiometry during treatment in SHL patients. Both AECA-positive and -negative subjects received 10 mg/d venous dexamethasone for 5 days followed by gradual tapering of dose of 5 mg/d for another 5-day. Then pure-tone average was reexamined. Differences in hearing recovery between AECA-positive and -negative subjects and relationship between AECA level and hearing recovery were analyzed.@*RESULT@#The prevalence of AECA detection was 68.75% (33 of 48 patients) in SHL patients, with significant difference compared with control subjects with 23.33% (7 of 30 controls) (P<0.01). After treatment, rates of response to dexamethasone in AECA-positive and -negative SHL patients were 81.8% (27 of 33 patients) and 33.3% (5 of 15 patients), respectively. Meanwhile, there was a significant difference in cure, excellent recovery, partly recovery and invalid between AECA-positive and -negative groups [21.2% (7/33), 33.3% (11/33), 27.3% (9/33) and 18.2% (6/33) versus 0, 13.3% (2/15), 20.0% (3/15) and 66.7% (10/15), P<0.01]. Except 5 subjects with AECA level more than 263 microg/L, hearing recovery was correlated to pretreatment AECA level (r=0.8084, P<0.01).@*CONCLUSION@#In sudden HL patients treated with dexamethasone, AECA might represent a serological marker of prognosis.
